Wellcare has partnered with Healthmap Solutions (Healthmap)†, a kidney population health management company, to help provide care for members with chronic kidney disease (CKD) and end-stage renal disease (ESRD) in addition to co-morbidities such as high blood pressure, diabetes, or heart disease. Healthmap’s Kidney Health Management (KHM) program works with you and your doctors to help reach your health goals.
If you qualify for the KHM program, it will be offered to you at no cost. As part of the program, you will receive support from our Care Navigation team, which includes nurses, wellness coaches, dietitians, and social workers.
